Course Designation Form

To begin taking courses towards the Combined Bachelor's/Master's Degree, students are required to submit this Course Designation Form before enrolling in any undergraduate course which is to be considered for graduate credit. (Students applying to Mechanical Engineering are not required to submit a course selection sheet. For more information, ME students should see the Graduate Catalog.) The form must be approved by the department head or Graduate Coordinator, as well as the individual instructors for each course. Copies of this form will be sent to the department and the Office of the Registrar, with the original staying with the graduate admissions record.

Instructions for Completing the Course Designation FormFor Students Filing an Initial Course Designation Form

  1. Please complete Sections I and II of the Course Designation Form.
  2. Please obtain a signature from the instructor for each course listed in Section II.
  3. Once all instructor signatures have been obtained, a final signature from the Department Head or Graduate Coordinator is required.
  4. Please submit the Course Designation Form to the Graduate Studies & Enrollment Office before the start of any courses listed on the form.

For Students Amending a Previously Submitted Course Designation Form

  1. Please complete Section I and list any courses you wish to add in Section II.
  2. Please obtain a signature from the instructor for any course(s) you are adding in Section II.
  3. Please complete Section III by listing all previously approved courses and indicating for each if you intend to retain the course or remove the course.
  4. Once all instructor signatures have been obtained, a final signature from the Department Head or Graduate Coordinator is required.
  5. Please submit the Course Designation Form to the Office of Graduate Studies & Enrollment before the start of any courses being added.
